The Rapid Equipment Tracker app (also known as the Form 37 app or CAPF 37 app) is an application built on CAP's D2R platform to help members track temporary issues of CAP equipment. It is intended to decrease costs of equipment loss by making it easier to track temporary issues of equipment.

The app is located at https://form37app.mendixcloud.com/

To log in for the first time, use the 'Forgot Password' feature.

Features

The Form 37 app supports the tracking of these operations:

  • The temporary issue of equipment from one member to another (and return of equipment)
  • The temporary issue of equipment from an equipment locker/bank to a member (and return of equipment)
  • See the timeline of temporary issue transfers (NOTE: this feature is available only to commanders, logistics officers, supply officers, transportation officers, and communication officers)
  • (coming soon) Search for any equipment nation-wide and find contact information for the equipment's custodian
  • (coming soon) Track equipment for a large event or exercise (like NESA or wing conferences)

It can track both non-expendable equipment (equipment purchased for $500 or more, which exists in O.R.M.S.), and expendable equipment (equipment purchased for less than $500).

Limitations

  • Lack of single-sign-on. Currently, all of the app's logon usernames/passwords are independent from eServices.
  • The form 37 app doesn't write information to ORMS; it maintains a completely independent copy of data

Known Bugs

  • When equipment is returned, it creates another Form 37 for the return, and the 'return Form 37' remains on the returnee's list of temporarily issued equipment
  • Official/non-temporary issue transfers are not reflected in the app
  • The CAP regulation governing equipment accountability (CAPR 174-1, which is found on CAP's Regulation page) does not mention, require use of, or promote use of the Form 37 app

Releases

The app was released for general use on 25 Sep 2024. At this time, it is open to all CAP units who wish to use it.
